    Nursing Services

    Our licensed, experienced nurses are on duty 24-hours a day to provide direct nursing care when needed.  RN coverage at Pioneer Memorial Care Center exceeds the regular coverage at other organizations as there is RN coverage during evening shifts.

    Physician Services

    A Physician is on call 24-hours a day.  Our Medical Director is contracted through Essentia Health of Fosston, MN.  Each resident is given a monthly visit to assess health needs and Pioneer Memorial Care Center also works closely with Rural Psychiatry for Psychiatric evaluations on a monthly basis if deemed necessary.  It is the residents' and familys' choice to elect their Primary Physician and we will gladly assist in scheduling visits. 

    A beautiful courtyard and picnic area overlooks the golf course. 

    Food and Nutrition Services

    We have a full-time Registered Dietitian of Food and Nutrition Services, as well as qualified and well-trained staff to take care of resident’s food and nutritional needs. The facility features four community dining areas where three meals are served daily.  The dining rooms are designed as if you were at a restaurant to promote social interaction among residents, families, and friends.  Between meals, residents can enjoy a variety of beverages and snacks throughout the day.  Special meals and treats are also incorporated through our activity program throughout the week.  Our scenic courtyard overlooks the Oak Lake Golf Course which is available for family picnics and cookouts.

    Social Services 

    We have a full-time Social Services Designee to meet the psychosocial needs of the elderly, recovering, and disabled residents to help contribute to their overall quality of life.  Social Services is available to develop action plans, monitor and develop care plans with the interdisciplinary team and deliver resources to meet the needs of the residents and their families.  PMCC will assist in building on family involvement in the residents' lives, as well as that of the staff.  Pioneer Memorial Care Center prides ourselves in maintaining an open line of communication with families at all times. 

    Rehabilitation and Restorative Services

    We are contracted with Altru Health System's Outreach Therapies for occupational, physical, and speech therapy services.  All therapy services are performed at the Care Center while inpatient and subsequent to a transition home or to assisted living, outpatient services can be performed by the same therapists at the attached Altru Clinic of Erskine.  Goals are established based on multidisciplinary evaluations completed at the Care Center with input by physcians, nursing staff and family members, as well as the resident.  Treatment focuses on improving or attaining independence in functional abilities within Pioneer Memorial Care Center and applying those for a possible return home or to another supported living facility.  

    Housekeeping Services

    We provide a clean, safe, and comfortable environment with our daily housekeeping services provided on all wings.  Personal laundry services are provided on the individual communities to ensure the best care for the residents' clothing and bedding.  Dry cleaning services are also available via request. 

    Dental & Eye Care Services

    Each resident has their choice of dentist and optometrist.  Our Social Service Designee and Nurses will assist in the scheduling of appointments and contact family regarding the outcomes of the visit.

    Resident Council & Resident Care Conferences

    Pioneer Memorial Care Center has an active Resident Council, comprised of residents within the Care Center.  The Council meets monthly to express ideas, wishes, discuss matters of concern, and other details regarding their stay with us.  Resident Care Conferences are held every 90 days for each resident with our interdisciplinary team. The family and residents are encouraged to attend to review the resident’s status, outline future goals, and discuss expectations.
