    Pioneer Memorial Care Center / The Country Place / Crossroads


    To Our Residents, Family Members and Employees:

    We want to inform you that at Pioneer Memorial Care Center, from our Outbreak testing that was completed on 11/3/20 almost all residents testing results in the nursing home have came back negative and currently no new positive nursing home staff.

    Our Assisted Living facilities completed an Outbreak testing due to a positive employee on 11/5/20 and have resulted in 5 positive tenants and 1 employee at this time. Families and tenants have been notified, and all have been put on quarantine as directed by MDH and CDC.

    We are still waiting for a few results to come in but wanted to make you aware, community spread is continuing and we need to remain vigilant. This will be our new norm it appears and so ensuring that each individual follows the recommended guidelines will help our community with the spread.

    Our positive employees are at home following CDC guidelines and the employees from the facility who have tested positive are currently in the process of contact tracing with our Infection Preventionist. Our facility will continue to be listed on the Minnesota Department of Health (MDH) website as a COVID positive site due to the employee's illness.

    The safety and wellbeing of our residents continues to be our top priority. We are doing what we can to limit the spread of COVID-19 within Pioneer Memorial Care Center, including staying in very close communication with local and state health officials to ensure we are taking all the appropriate steps under current circumstances.

    We are committed to keeping you informed. If we experience any new cases, we will provide an update to you.

    We are taking steps based on guidance from the Centers for Disease Control and Prevention (CDC) and the Center for Medicare and Medicaid Services (CMS) to reduce the spread and impact of COVID-19, such as:

    • Enhanced infection control precautions

    • Continued screening residents, staff, and essential visitors for expanded list of symptoms

    • Continued restrictions visitation and entry of people to the building

    • Testing staff and residents for COVID-19 based on current protocols and availability of tests

    Our obligation is to ensure the safety for both staff and residents by following regulatory guidance. Employees are required to maintain the privacy of medical information and the privacy of all individuals. We ask that families of our residents to do the same to keep protected information private and avoid spreading panic unnecessarily.

    We also understand that connecting with family members is incredibly important to our residents. Family members are encouraged to connect with their loved ones through video chat, calling, texting, or on social media. Pioneer Memorial Care Center has multiple I-Pad type devices available for video chatting. If you would like to utilize this option, please contact Activities to schedule a time and we will gladly make arrangements.

    We will continue to share information as it changes and as we are allowed through MDH guidance. Please keep yourselves safe by following social distancing and washing your hands often. We know that you may have questions and we encourage you to contact our center at 218-687-2365 or email us at

    We will be completing testing again for all residents and staff Tuesday November 10th, 2020 and Friday November 13th, as our country rates require bi-weekly testing.
